- Select the restaurant location you visited
- Enter the transaction number or check number from the receipt.
- Answer the questions honestly and rate your satisfaction level
- Fill out the rest of the survey
- Provide the required details e.g. Name, Address, Phone Number, Email etc. and submit to enter the sweepstakes to win $300 Mooyah Gift Card automatically
